Create a New User Account by clicking on the New Users Click Here to Register link. Register by entering your information in the applicable fields (there are a few mandatory fields) and create a User Code and Password. Once complete, make sure to click the 'Save' button.

When registering, please update your:

• Allergies List
• Current Medications List
• Past Medical History (Found in Forms Link)
• Insurance Records
• Click here and complete the attached form to release your records to Walker Family Medicine.
