- Will you accept my insurance plan?
We are working very hard to get on all of the insurance plans in the area.
- How do I transfer my records to your practice?
You mat transfer records to our practice by completing and mail us this form.
- Are you accepting new patients?
Yes, we are currently accepting new patients. To become a new patient simply sign in as a new user in the Patient Center and complete all the the forms listed.
- How do I know how my medical information will be handled by Walker FAmily Medicine?

- How do I establish or re-establish my relationship with a doctor at Walker Family Medicine?
In order to become a patient at Walker Family Medicine you must have never been forcibly discharge from Dr. Walker's, Mick Drage's, or Amanda Bennett's practice in the past. If this is the case, and you wish to be a patient with Walker Family Medicine, simply complete the new user sign in in the Patient Center, make an appointment, or notify us that you wish for one of our providers to provide your medical care. Then complete and mail us Medical Records Release Form.
- I am an established patient of Dr. Walker, Mick Drage, or Amanda Bennett at SSMC or SMC. Will you allow me to continue to be your patient in your new clinic?
Yes! We appreciate and cherish the trust you have placed in our medical services. We are accepting new patients and we certainly are very pleased to have the opportunity to continue our relationships with our current patients.
- Will you accept walk-in patients?
Illnesses and injuries are not something that are or can be scheduled. If you become ill or injured and need to see a doctor we will get you seen on the day that you are sick or injured.
- How do I attain a copy of my medical records from Walker Family Medicine?
Simply login to Patient Center and request the release of your medical record. This is done in the Appointment section where the link is "Release of Information" near the bottom of the page. |
